Access To Records

In accordance with the Data Protection Act 1998 and the Access to Health Records Act 1990 - patients may request appointment to see their records and may be subject to an administration charge.

No information will ever be released without your consent unless we are legally obliged to do so.

In return we ask you to...

Ensure that your written consent accompanies any request you have authorised for 
medical information.

Make an appointment with the doctor if you wish to see your records.

Consent And Confidentiality

Time to time you will be asked to provide consent for disclosure of your medical records or Medical reports by agencies e.g. : your employer or insurance company etc.

Please make sure you read care fully before signing and if you are not sure or wish to see the report contact the surgery and let us know .

We do not disclose any of your information without your accompanied consent.

Be aware that we have contractual obligations to provide information to Health authorities for financial auditing , research etc. and some times to the external agencies with view to provide or improve the service to you without disclosing your personal identity.
